Ghost perk/vest prevents notification. And ghost vest is pretty popular rn. I wonder if they patched it cuz from what I've seen the stealth vest still activates the comms vest, it just makes you invisible on radar. Doesn't matter anyways, I had to make a new one and it works fine now. That should not exist.

Switch to stuck operator. Finish a contract and exfil. Sometimes the exfil streak gets bugged. Failing to exfil or changing operator in that slot will fix it (by resetting it to zero so you can start again). Some people have had luck fixing it by changing operator skin and/or finisher. Basically: good luck.
